The face can be thought of as made up of three layers: the skin, the outermost layer; the underlying soft tissues, which include fat and muscles of facial expression; and finally the bony foundations of the facial skeleton and its ligamentous anchors. The ageing face sees changes in all these components, which often occur at different rates in these different components and anatomical subsites. Atrophy of subcutaneous fat, recession of bony landmarks and laxity of ligaments, for example, allow unsupported skin to fall into wrinkles and folds, and for surface irregularities to develop. Coarse textural changes and alterations in pigmentation may be the result of external assault of the environment, especially the sun not to mention the passage of time.
Traditionally, facial rejuvenation focused on invasive procedures, such as ablative laser resurfacing or facelift surgery, that may be associated with expected periods of recovery, some patient discomfort and possible complications. A trend in public expectations for minimally invasive procedures with appreciable results and fewer side effects over the last two decades has led to a surge in popularity of ‘lunch-time’ treatments, from botulinum toxin to soft tissue injectable fillers which address some of the concerns of an ageing face. Clearly, these have a growing place in maintenance and slowing down of the ageing face as well as rejuvenating areas of concern.
Botulinum toxin is derived from Clostridium botulinum, the bacterium identified as the causative agent of botulism (from contaminated blood sausages) in nineteenth-century Germany. Botulinum toxin A was originally isolated, purified, concentrated and crystallized for use as a potential biological weapon in 1946. In recent years it has found its commercial use as an anti-wrinkle treatment. By binding to the motor nerve end terminals it results in temporary paralysis of muscles. Injection into the muscles of facial expression responsible for exacerbating forehead, glabellar (between the eyebrows) and periocular (around the eye) wrinkles will produce results lasting 3 months. Botulinum toxin can also be used to treat hyperhidrosis (increased sweating) with with good results.
In recent years anti-wrinkle injections administered by a variety of health workers have become commonplace because it is believed that such treatment can be carried out without expert training. Dr Farhadieh believes any such procedures should be undertaken within the sphere of those with facial aesthetic surgical training and expertise. Any facial procedure can be thought of as a exercise of a nuanced surgical art. Any action can be thought of as lying on a continuum that starts with the simplest personal sun protection and moisturizing cream across the spectrum of injectables, to ablative lasers, facial peels and finally ends with complete facial rejuvenation surgery.
